Summer Knights Event: $40

What is Summer Knights?

The event will take place Friday, August 8, and typically starts at 5:30 pm and ends at 8 pm.  It will be a fun evening for the kids to hang out, eat, play games, and play in the Dodgeball tournament with a Grand Trophy prize for the winner. All students that register for the event will receive a wrist band for entry as well as a second wrist band for those in the dodgeball tournament.


Meal to be served:

Chuy's meal will be non-vegetarian or vegetarian and will include chips, seasoned ground beef, refried beans, queso, Boom Boom sauce, Salsa Fresca, and Creamy Jalapeño. We will also have additional condiments for the nachos like sour cream and jalapeños. Ice cream cups for dessert.


Waiver to sign below:

I am the Parent/Guardian of the Fort Bend ISD Elkins Fine Arts Student listed above, and/or the Student listed above. I understand and agree, in the event of any injury to Student during this challenge, I will not hold Fort Bend ISD, Elkins Fine Arts, its directors, booster club, booster officers and members, responsible for any decision relating to the medical treatment for the above Student or for such treatment itself.


I hereby fully waive, release, indemnify and hold harmless Fort Bend ISD, Elkins Fine Arts, its directors, booster club and booster club officers, members, district employees agents and assigns from any and all liability, loss, damage, injury, claim of any nature whatsoever arising out of or in any way related to my student/my participating in the Summer Knights activities, including dodgeball. 


Participating in any activity is an acceptance of some risk of injury. I/my student assumes all risks related to the activity. In case of emergency, I do hereby authorize and consent to any medical treatment or care deemed advisable to me/my student.


I have read and agree to comply with all rules and regulations.


My signature (typing in my name) here indicates that I have read, understand and have freely signed this agreement. My signature below indicates that my student has permission to participate in Summer Knights activities.


If paying by Credit Card:

If paying by Zelle: 

Select “Pay Later,” fill out the form, and add to cart. At check-out, transfer total fees to the email address: elkinsbandzelle@gmail.com. Use the name Elkins Band Booster Club. Put your student's name and "Registration Day." (e.g. Sue Smith - Registration Day). Zelle: elkinsbandzelle@gmail.com. Provide Zelle confirmation number to parent for the registration sheet.You will get an invoice, but you don't need to pay it since you already paid with Zelle.


If paying by Check:

Select “Pay Later,” fill out the form, and add to cart. At check-out, make the check out to "Elkins Band Booster Club" and submit then or send to school with your child to give to Mr. Chen. You will get an invoice, to serve as your reminder to send in the check if you haven’t. DUE JULY 31.
